I have setup a private stellar network for testing. I am trying to submit a payment transaction and am getting the error [tx_insufficient_fee]. However, my source account have more than enough fees to cover it.

In fact, I tried submitting the transaction again, it worked. What could be causing the transaction to sometimes fail with insufficient fee?
To add: I'm getting the error whilst doing load testing on my private network, submitting multiple transactions from multiple different accounts at the same time. Not sure if this will affect the fees?
I am setting the fee for each transaction = [number of ops] * 100
